We are looking for a committed, caring, and reliable Director of Social Service to join our in-house rehab team for our Skilled Nursing Facility!

RESPONSIBILITIES:

• Develop and implement strategic plans and initiatives to improve the delivery of social work services
• Oversee the day-to-day operations of the social service department, including budgeting and resource allocation
• Recruit, train, and supervise a team of social service professionals, ensuring high-quality care and service delivery
• Collaborate with interdisciplinary teams to develop and monitor care plans for clients, addressing their social and emotional needs
• Conduct assessments and evaluations to identify clients' strengths, challenges, and goals
• Provide direct social work services to clients, including counseling, crisis intervention, and advocacy
• Foster and maintain effective relationships with community organizations and resources to enhance social support for clients

REQUIREMENTS:

• Bachelor's degree in social work or related field; master's degree preferred
• Minimum of 1 year of experience in social work, preferably in a healthcare setting
• Strong knowledge and understanding of social work principles and practices
• Proven experience in program development and evaluation
• Excellent leadership and team management skills
• Exceptional communication and interpersonal abilities
• Ability to multitask, prioritize work, and meet deadlines in a fast-paced environment

EXPERIENCE:

• With Skilled Nursing or Long-Term Care experience is preferred
• Experienced Director of Social Service
• Similar leadership roles in a healthcare setting are welcome
