Full Job Description: Case Manager for Residential Treatment Program

Position: Case Manager

Location: Lowell Area

Salary: $19.00 - $22.00 per hour (Dependent on Experience)

Daily Life: The schedule is in person from Sunday to Thursday. Schedule is flexible with some day and evening shifts. As a Case Manager, your day will involve meeting with clients in residential programs to assist with various aspects of their rehabilitation. Your responsibilities will include supporting clients in medical appointments, aiding in job search services, driving clients to appointments, and collaborating with the treatment team to support client progress.

Requirements:

• Bachelor's Degree preferred, those with Associates and strong experience will be considered
• Minimum of one year of experience in a case management role
• Active and unrestricted driver's license

Responsibilities:

• Assess the residents' needs concerning vocational or educational services.
• Provide coaching to residents on resume writing, job searching, educational opportunities, budgeting, and aftercare.
• Complete accurate documentation in a timely manner, in compliance with licensing requirements and Foundation policies and procedures.
• Identify and connect residents with community resources that support growth and independence.
• Facilitate psychoeducational groups.
• Establish, coordinate, and transport residents to necessary appointments.
• Provide aftercare follow-up calls and support, while maintaining thorough documentation.
• Complete ESM (Engagement, Stabilization, and Moving Forward) forms for each assigned resident.
