The Care Coordination Supervisor is accountable for developing, coordinating, and guiding ACHN or ICN staff activities related to daily care coordination functions.
ICN - This position will serve as a resource to assist the care coordination staff, as they provide care management services to enrolled members. This position directs the work of Home and Community Based Services (HCBS) case managers as they manage members to improve their health conditions. The Care Coordination Supervisor may interact with Physician offices, agencies, case management organizations and facilities when questions or concerns arise regarding the referral process or a member's plan of care. This positon indirectly oversees all care coordination documentation for ICN activities.
