Speech and Language Pathologist - Home Care assesses and treats speech and language disorders and impairments in a home care setting. Conducts evaluations of an individual's speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Home Care develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating, and counseling. Additionally, Speech and Language Pathologist - Home Care completes required documentation to record assessment and progress. Collaborates with physicians, family members, and other clinical providers to provide effective therapies. Ensures that treatment that is provided aligns with patient needs and is within the guidelines set by the payer. May direct support staff in assisting with treatment. Requires a master's degree in speech pathology. Typically requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP). Requires State License based on Clinical Specialty. In this role, you will be responsible for performing evaluations of communication skills and swallowing functions and provide treatment plans that allow your patients to reach their maximum communication and swallowing potential. You will work cooperatively with members of an interdisciplinary team to identify and solve patient needs with quality care. You will not only play a major role in your patient's progression, but will also provide them, along with their families, the resources and education needed for a successful transition out of our care.
