Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Phoenix Hospice Admissions RN assess and determines hospice appropriateness of patients seeking end of life care. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, communication, and multidisciplinary approach that encompasses the physical, but also the spiritual and psychosocial needs of the patient and their family.
