Elite Professionals is hiring for RN LEADERS, your job as an admissions focused RN is to review, assess, and assist patients as they begin their Home Care Journey. Your primary focus is to assess and complete a thorough admission to our agency, identify special programs within Élite that will assist patient in improved health, create the Plan of Care, provide recertification's for care as needed, discharge, resumptions of care if patient hospitalization occurs, Staff supervision, education, and plan of care creation.

We work closely with our client’s physicians to provide the highest quality of care possible. Each client will receive individualized and superior levels of care. Elite offers to our clients: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Work Services, Lymphedema Services, Behavioral Health Nursing and Home Health Aide assistance.

ORIENTATION FOR THIS POSITION

• 3 Days in office required (Mon/Tue/Wed) 9am-4p
• 2 Partial Days week days (flexible time)
• Will need to shadow current staff during weekend hours for field orientation

• Elite Professionals Home Care LLC is looking to add RNs to our team. Every other weekend needed.
• Can include Mon-Thurs On Call evenings for additional pay
