Strategic Planning Director directs and oversees an organization's strategic and long-range goal planning function. Drives strategic initiatives and supports the development of long-term growth plans and profitability goals. Being a Strategic Planning Director analyzes emerging industry trends, expansion opportunities, including mergers and acquisitions, competitive threats, the viability of outside business partners, and sources of capital. Establishes and retains relationships with industry analysts and the investment community. Additionally, Strategic Planning Director utilizes data-driven methodologies to analyze business performance metrics and process improvement options effectively. Develops reporting and tools to communicate strategic plans and concepts to leadership. Conducts periodic status reviews of organizational goals and objectives to identify roadblocks to progress. Requires a bachelor's degree. Typically reports to top management. The Strategic Planning Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Strategic Planning Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Facilitates cost-effective, quality patient transitions of care and outcomes by collaborating with staff and providing guidance, education, and support to assigned patients.
     
    Job Responsibilities
    • Completes comprehensive assessment of medical record and knowledge of patient's medical condition, financial status, emotional needs, and support systems.
    • Collaborates with Case Management Medical Director, Director, and/or Manager on complex discharge planning issues and delays in service.
    • Interview patients, assesses priorities, and develops appropriate discharge plan.
    • Works in collaboration with the Social Worker and interdisciplinary care team members to ensure quality support of care and safety of discharge.
    • Assists patients/families with referrals to community agencies, post acute providers, and provide education and resource information.
    • Documents precisely and timely in the medical record the assessment, concerns, needs, and discharge plans.
    • Provides ongoing assessment and keeps plans up to date as patient progresses toward discharge.
    • Trains new employees, students, or interns as needed.
    Qualifications
    • Associate's degree or higher or Diploma in Nursing required upon hire AND
    • Bachelor's degree or higher in Nursing required within 5 years of employment in this position
    • No experience required
    • RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire
