    Responsibilities:

    • Complete individual resident assessments and care plans upon admission/quarterly/annual and as needed
    • Coordinates, implements, and monitors all service plans and other state required documents with other members of the team where appropriate
    • Assures implementation of all policies and procedures.
    • Performs ongoing quality improvement evaluations that include quality indicators for the environment, documentation, team functioning, and all aspects of quality care provision as well as reporting identified issues back to the team for resolution.
    • Coordinates and collaborates with other team members to promote inclusion of patient in unit activities as appropriate.
    • Engages outside resources to complement activity program (e.g., entertainment, intergenerational programming, virtual memory exercise programs etc.).
    • Assists with personal care as needed.
    • Maintains up-to-date knowledge and skill in specialty to ensure quality programming for residents.
    • Participates in team and staff meetings to ensure coordination of quality customer care.
    • Develops daily and monthly activity dementia calendar for the unit that includes both small and large group activities and 1:1 programming.
    • Maintains and implements daily completion of activity participation logs
    • Participates in other Program efforts as needed
    • Maintain and oversee team members schedule and job duties (activity assistants) during shifts
    • Understanding that some nights, weekends, and holidays are required to work
    • Work and collaborate with the Administrator to build and enhance the overall Activity Department and the well-being of all residents
    • Knowledge and understanding that this is a hands-on position and are expected to help programs run smoothly, transport residents to and from programs and set up or break down programs as needed.

    Here's what's needed to be part of this amazing team:

    • Graduate of an accredited college or university with a bachelor's degree (therapeutic recreation, psychology, gerontology, or another related field preferred but not required)
    • Three years of direct experience (in respective discipline) with dementia and/or behaviors
    • Prior experience as an Activities Director preferred
    • Two years of supervisory experience in an appropriate healthcare setting.
    • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served
    • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
    • Certified Dementia Practitioner and/or Memory Care Certification preferred but not required
