Coordinates clinician schedules and assigns clinicians to patients based on the frequency and expected duration of prescribed treatments and therapies. Being a Team Leader - Home Care monitors utilization, productivity, and tracks patient outcomes. Ensures compliance with agency policies and procedures. Additionally, Team Leader - Home Care typically requires a bachelor's degree. May require an RN license. Typically reports to a director. As a Lead Caregiver at Trail Ridge Home Care, you'll help to orientate new team members, provide mentorship and training to the caregiver team as requested, and assist in filling in client assignments and on-call rotation as needed.
You'll also work in a caregiver capacity, providing companionship and assistance with personal care. You'll work directly with seniors in their homes using a variety of techniques to encourage and support their ability to maintain or attain their highest level of physical, mental, and social well-being. You'll take pride in managing the needs and care regimen of your clients and partner closely with our courteous business office staff to develop care plans.
