  • Brief Summary:

    Under the supervision and guidance of the Chronic Disease Management Provider, the Chronic Disease Management Outcomes Coordinator is responsible for the collaborative process of assessment, planning, implementation, and evaluation of care coordination for patients. The coordinator advocates for options and services to meet and individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost effective outcomes. The coordinator demonstrates knowledge and skills and understands the unique aspects of the patients served. In addition, perform nursing care and case management for patients with the collaboration of Samuel Simmonds Memorial Hospital (SSMH) Chronic Disease Management providers in the clinic, procedural and inpatient setting including but not limited to acute care and swing bed admissions.

    Essential Job Functions:

    • Under the supervision and guidance of the Chronic Disease Management Provider, provide care and coordination for patients.
    • Facilitate care coordination across the care continuum.
    • Make client appointments and travel requests.
    • Collect data, signs, and symptoms of deviation from normal health status.
    • Monitor patient condition and report deviations from normal health status to the provider
    • Document patient information in the electronic record.  
    • Maintains all required certifications and licensing required for employment.
    • Maintain regulatory requirements regarding confidentiality and patient/client and family care including all federal, state, and Joint Commission and HIPAA requirements.
    • Assists leadership in Chronic Disease Management program development and continuous improvement through measurement and feedback of appropriate outcome-based processes and strategies. Actively participates in developing program structure, tools, procedures and communication strategies.
    • Management of measurement development, data collection, monitoring, analysis and reporting to support desired program outcomes. Displays innovation in problem solving.
    • Other duties as assigned.                                                       

    Licensure/Certification/Registration:

    • Current Alaska Registered Nurse License or another US state RN license with ability to obtain AK RN license within six (6) months of hire.
    • Current BLS Certification. ACLS preferred.
    • Case Management Certification preferred.

     

    Education & Experience:

    • Graduate of an Accredited School of Nursing.
    • Able to articulate and demonstrate appropriate nursing theory and practice.
    • Two years recent nursing experience required.
    • Case management experience preferred.
