    Community Health Worker Job Responsibilities:

    • Assists clients in their homes, community, or clinic setting. Communicates to patients the purposes of recommended treatments and the impact it may have on their wellbeing.
    • Serve as a role model and share personal insight into healthcare in effort to motivate and support patients.
    • Coach patients in effective management of their chronic health conditions and self-care. Assists patient in understanding care plans and instructions. Motivates patients to be active and engaged participants in their health and overall wellbeing.
    • Assists clients in accessing health related services, including but not limited to: providing instruction on appropriate use of the medical home, pharmacy, Emergency Room/Hospital, specialist services and helping overcome barriers to obtaining needed medical care and /or social services.
    • Assist in linking patients and families to social/other services for identified needs and provide follow-up to ensure successful transitions.
    • Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed. Follows up with both clients and providers regarding health/social services plans.
    • Continuously expands knowledge and understanding of community resources and services. Facilitates patient access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services.
    • Assists clients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
    • Assist patients as a connector obtain insurance in the marketplace and work with all uninsured patients obtain appropriate coverage.
    • Propose quality improvement and policy changes that optimize patient care transitions and improve health outcomes.
    • Will be trained and checked off to help in clinic as needed for triage or other clinic duties, including front office check in.
    • Will attend Outreach Events, and Health Fairs as scheduled by Supervisor.
    • Document details of direct patient care, phone calls to/from patient, and mail correspondence correctly in-patient chart.
    • Work with the Outreach Coordinators to establish and to maintain a relationships with local business and community partners
    • Collaborate with Community Partners to identify outreach opportunities to serve BCHC’s Special Populations (i.e. farmworker outreach clinics, street medicine, flu vaccine clinics, health fairs, and community events).
    • Promote and provide vaccinations in the community, including COVID, tdap, Monkey Pox and Flu vaccine.
    • Acts in compliance with contracts, laws, regulations, BCHC policy, procedures, HIPAA policies and standards.
    • Engage in continuing education to advance knowledge and understanding of the needs of BCHC’s special populations.
    • Conduct assessments of the needs of BCHC’s special populations: migrant & seasonal farmworkers, individuals experiencing homelessness, and recently resettled refugees.
    • Provide enabling services including health education, care coordination, and case management to connect patients with referral sources (i.e. transportation, housing, clothing, and food).
    • Document all patient encounters in BCHC’s electronic medical record in a timely manner.
    • Conduct health screenings and education in outreach settings including vital signs,
    • blood glucose, HIV screening/counseling, SBIRTs, SDOH and others as requested
    • Complete special duties or projects as assigned.
