JOB SUMMARY:

Provides quality professional services in the home care setting as prescribed by the physician.

JOB FUNCTION:

• Responsible for ongoing interdisciplinary assessment and prepares clinical notes
• Develops and evaluates the plan of care with the patient, patient representative, or caregiver, provision of ordered services, patient, caregiver education,
• Responsible for ongoing interdisciplinary assessment.
• Supervision of LPNs
• Communicates with all physicians and health care practitioners involved in the plan of care
• Participates in the QAPI program
• Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental infection control standards

JOB QUALIFICATIONS:

Education:

· High School Diploma or equivalent

· Graduated from an accredited school of Nursing

· Ongoing participation in Allen Parish Home Health in-service training

Job Knowledge:

· 1 year experience in home care is preferred.

· 2 years’ experience in Med-Surg is preferred. (if none, will provide training with LPN or RN)

License:

· RN license in state of LA is required

· Valid Driver License and Auto Insurance is required

· CPR/BLS certification is required

Job Type: Full-time

Pay: $27.86 - $41.80 per hour

Expected hours: 40 per week
