Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral health services for children and adults, and substance use disorder treatment, and specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. Under the direction of the Director of Medical Operation, the Community Health Worker provides outreach and care coordination to the identified population and coordinate administrative requirements per the contact with the Connecticut River Valley Farmworker Health Program (CRVFHP). Activities will include direct outreach to the target population and local farm owner/operators to ensure access to health care, enrollment into the program per guidelines set forth by CRVFHP, coordination with outside specialty providers to ensure access and proper coordination of benefits, and regular data gathering and reporting as required by the program.
