Establishes accessible systems that respond to patient needs for social services including counseling, financial, bereavement, and specialized support during and after hospitalization. Establishes relationships with outside agencies and charitable organizations to leverage additional sources of support for patients. Responsibilities: - Provide direct care and support to individuals with developmental disabilities or seniors in a residential setting - Assist with daily living activities such as bathing, dressing, grooming, and toileting - Administer medications according to prescribed schedules and document accurately - Prepare meals and assist with feeding if necessary - Support individuals in participating in recreational activities and community outings - Promote independence and encourage the development of life skills - Ensure the safety and well-being of individuals at all times - Maintain accurate records and documentation of services provided Skills: - Experience working with people with developmental disabilities or seniors preferred - Knowledge of direct support techniques and person-centered care approaches - Ability to drive and transport individuals to appointments or activities - Familiarity with dementia care principles is a plus - Strong caregiving skills, including empathy, patience, and compassion - First aid and CPR certification is desirable - Proficient in meal preparation and following dietary restrictions - Understanding of medication administration procedures - Knowledge of resident rights and advocacy We offer competitive pay rates, flexible scheduling options, and opportunities for career advancement.
