Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. This newly created administrative position reports to the Clinical Manager of Hospice and Palliative Care. The Hospice Coordinator is responsible for providing clerical and administrative support necessary to assure delivery of hospice clinical care for our patients and staff, coordination of scheduling for the Hospice team including admissions and visits for the clinical team and home health aides. The Hospice Coordinator role is responsible for coordinating data entry of documentation for the Hospice team and communication with patients, physicians, and staff. Demonstrates proficiency with the electronic medical record. The Hospice Coordinator will adhere to HIPAA/WISP policies to maintain the confidentiality of PHI. RESPONSIBILITIES:
· In conjunction with the Clinical Manager of Hospice & Palliative Care accepts referrals from Intake and assigns to case manager.
· Completes the admissions and visits scheduling for all clinicians and visits for home health aides based on a variety of factors. Manages staff member(s) geographical areas when scheduling visits.
· Performs review of the EMR to assure accuracy and completeness of data according to established guidelines. Notifies clinician and/or Clinical Manager regarding any concerns.
· Assigns patients to the appropriate hospice team members taking into consideration timeliness of admission, timeliness of ROC and appropriate discipline i.e. RN, LPN.
QUALIFICATIONS:
· High School diploma or equivalent required.
· Minimum two years office experience, preferably in a health care environment.
· Knowledge of medical terminology and prior scheduling experience preferred.
· Computer/data entry skills.
· Excellent organizational/communication skills.
· Able to handle multiple tasks.
· Adheres to Agency’s confidentiality policies.
· Exemplary customer service skills.
Ability and willingness to follow up on completion of all duties and team responsibilities.
