The successful candidate will be part of the patient's clinical team, responsible for managing the patient’s wounds by performing treatment visits and completing progress notes in a timely and efficient manner using a proprietary EMR, working from 1-5 days a week, seeing 8-20 patients a day.

**Job Summary:**
The wound care provider is responsible for the assessment, treatment, and management of acute and chronic wounds in patients across diverse healthcare settings. Working collaboratively with physicians, nurses, and other healthcare professionals, the wound care provider develops and implements comprehensive wound care plans tailored to each patient's needs, with the goal of optimizing wound healing and preventing infections.
**Key Responsibilities:**
1. Conduct thorough assessments of wounds, including wound size, depth, appearance, and surrounding tissue condition.
2. Identify underlying causes of wounds, such as vascular insufficiency, pressure, diabetes, or trauma, through patient history review and clinical evaluation.
3. Develop individualized wound care plans based on assessment findings, patient medical history, and established wound care protocols.
4. Perform wound debridement, irrigation, dressing changes, and other specialized wound care procedures using aseptic techniques.
5. Monitor and evaluate wound healing progress, documenting changes in wound status, response to treatment, and any complications.
6. Educate patients, families, and caregivers on proper wound care techniques, prevention strategies, and lifestyle modifications to support healing.
7. Collaborate with interdisciplinary healthcare team members to coordinate care, address patient needs, and ensure continuity of care across different healthcare settings.
8. Stay current on advances in wound care technologies, treatments, and evidence-based practices through continuing education and professional development activities.
9. Participate in quality improvement initiatives, wound care research projects, and clinical trials to enhance patient outcomes and advance wound care knowledge.
10. Adhere to regulatory standards, infection control guidelines, and organizational policies and procedures related to wound care practice and documentation.
**Qualifications:**
- MD, DO, PA or NP with current state licensure.
- Minimum 1-2 years of clinical experience in wound care or a related specialty area.
- Strong assessment, critical thinking, and problem-solving skills.
- Excellent communication and patient education skills.
- Ability to work independently and collaboratively within a multidisciplinary team environment.
- Proficiency in electronic health record (EHR) documentation and computerized wound care management systems.
