The Disease Care/Coordination Nurse acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Being a Disease Care/Coordination Nurse requires an associate's degree/bachelor's degree, and is licensed to practice nursing.
