Job Description

The Home Care Pharmacist provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Being a Home Care Pharmacist adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Maintains all required documentation and data on prescriptions, inventory, and patients. In addition, Home Care Pharmacist requires a Ph.D. of Doctor of Pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Home Care Pharmacist's years of experience requirement may be unspecified.
