Abilities First, Inc., a CQL accredited organization, has been making a difference in the lives of people with developmental disabilities for over fifty years. Starting out as The Little Red School House in 1962, Abilities First, Inc. has evolved into the Hudson Valley’s premiere provider of support services for individuals with developmental disabilities. We offer a variety of services including school and preschool programs, day habilitation, residential habilitation, community habilitation, and employment services.

The Program Plan Coordinator (PPC) is responsible for ensuring the certification of documentation of each individual’s program plan. The PPC will act as a liaison with Abilities First providers, outside agencies, and the individuals in order to ensure regulatory compliance pertaining to documentation as it relates to paperwork and training for employment services.

Responsibilities will include but are not limited to:

  • Ensures the completion of written program plans and related objectives.
  • Responsible for ensuring participant’s data and related billing documentation using the designated software program is completed and reviewed in a timely manner.
  • Ensures compliance with all Community Habilitation and Respite billing, regulations, law and other requirements as it relates to paperwork.
  • Establishes a schedule with the Assistant Director for monitoring and oversight of required documentation.
  • Completes level one internal audits for Community Habilitation recipients.
  • Partner with the Assistant Director to assist with training.
  • Track, review, certify, scan, upload and distribute SAP/Life Plan/ ISP/ Com Hab Plan.
  • Primary liaison with Precision Care.
  • Will confirm that monthly notes are completed with accuracy and in a timely manner.
  • Provide direct Community Habilitation services to enrolled participants as needed to when other supports do ne exist.
  • Provide emergency phone coverage as per assigned schedule.

This is a full time position, 40 hours per week, Monday through Friday, 8:30 am -5:00 pm.

  • BA/BS in a human service field with one year’s related experience or a combination of education and experience working with developmentally disabled population or any combination of education and experience which in the view of the employer qualifies the candidate for the position may be acceptable.
  • Basic computer skills are required
  • Expertise in goal writing and implementation of individuals’ program plans is required
  • Must have and maintain a current NYS driver’s license, free of major moving violations; deemed acceptable by our insurance carrier.
