ACCESS COMMUNITY HEALTH NETWORK Community Engagement Coordinator Salary in the United States

The Community Engagement Coordinator utilizes innovative strategies and outreach techniques to educate, promote, and connect community members to ACCESS as their medical home. The Coordinator identifies and forms partnerships with community organizations that provide services to members of those communities.    What You Will Do – Top Priorities   Identify and facilitate community events such as community health and wellness fairs, church events, community workshops, senior events, and other community related events to educate community members on ACCESS’ services and programs and connect them to ACCESS for medical services    Develop and maintain partnerships with community organizations, coalitions, network groups, business leaders, and service providers with a strong focus on addressing health disparities and community wellness to establish ACCESS as a health provider of choice which results in referrals  Maintain record of outreach and engagement activities and submit information to supervisor upon request.  Work collaboratively with other ACCESS departments to support referral systems, health programs, and outreach events and initiatives ensuring all efforts are directed towards building ACCESS’ Medicare patient base  Support ACCESS patients and community members by scheduling appointments with appropriate ACCESS health center or directing them to internal systems to schedule appointments on their own.    What You Get:   An opportunity to impact the health of our patient population and wellness in our communities – You will be a vital part of communicating the mission of ACCESS to current and prospective patients! Freedom to utilize your tenacity and creativity to form new and maintain existing partnerships.   Required Qualifications:   High School or GED equivalent  A minimum of two (2) years of community organizing and/or outreach experience  Proven track record of community organizing and performing outreach at the community level  Ability to work a flexible schedule including mornings, evenings, and weekends  Intermediate proficiency with Microsoft office    Preferred Qualifications:   Knowledge of assigned engagement populations  Bilingual (Spanish/English)
