Position Summary   The Nutritionist/Dietician will provide quality nutrition information and education to clients and/or family members seen in an ambulatory care setting. Provide comprehensive diabetes information and education to adults, adolescents, and some pediatric patients with diabetes, in both individual and group settings.     What You Will Do   Advise and educate patients with dietary-related disorders on the practical ways in which they can improve their health by adopting healthier eating habits Promote healthy food choices and disease prevention by increasing awareness of the link between nutrition and health Assess and review treatment plan, including nutrition needs and meal planning, with patients, their physician and the diabetes educators as needed  Collaborate with a multidisciplinary team to perform a thorough nutritional assessment and education plan to assist in management of patients with diabetes and other chronic  diseases Develop and maintain nutrition curriculum and protocols, performing nutrition and educational assessments and providing patient education and family instruction  Manages the Diabetes Education Program, recognized by the American Diabetes Association. Ensure that all standards are being met and documentation consistently reflects the mission, vision and curriculum of the Diabetes Education Program as set by the American Diabetes Association     Requirement Qualifications   Bachelor’s degree Master of Science degree in nutrition strongly preferred. Valid Illinois license as a registered dietician Intermediate proficiency in Microsoft Office Suite   Preferred Qualifications   Master of Science degree in nutrition Experience in an outpatient diabetes center Minimum of two years’ experience as a certified diabetes educator Experience with American Diabetes Association recognition standards and program management Electronic Health Record (EHR) system experience, Epic a plus
