Specialized Case and Care Management Supervisor

This position is part of a contract granted through the Department of Social Services (DSS).  DSS identifies clients that are chronically homeless, having high costs for care and assigns them to this contract. The Specialized Case and Care Management team (SCC) and the OC DSS Liaison work in collaboration to prioritize referrals and maintain an informal wait list. The progress is monitored monthly in a meeting with DSS staff.  There are outcome measures that DSS has assigned to this contract and staff works to meet them on a 6 month basis.

These outcome measures include: increase in stable housing, connected to a PCP and completed an annual physical, reduction of medical and psychiatric ER visits, reduction in medical and psychiatric admissions, and minimum of 15 face to face contacts for each recipient per quarter.

The SCC program is fundamentally a relationship driven service where the connection between the case/care manager and the recipient is established and continuously reinforced.

The SCC Supervisor will supervise one case/care manager and will conduct individual formal bi-monthly casework supervision, support, guidance, and consultation. The formal supervision will entail detailed review of assigned cases, progress, risk management status, documentation compliance, issues encountered, concerns, and the ongoing assessment of the program outcomes for housing, healthcare, service access, quality of life improvement, and reduced costs.

The SCC Supervisor carries a caseload of 8 and the case/care manager carries a caseload of 12.

 Other responsibilities:

·         Initial assessment and engagement of people referred

·         Collaboration (and relationship building) with community service providers to address needs of the recipients

·         Planning for service delivery including methods and time frames

·         Utilization of evidence based practices (i.e. Motivational Interviewing, WRAP, Person Centered Planning)

·         Knowledge of TANF, SNAP, Medicaid, Homelessness resources

·         Data Collection / Outcome Reporting

·         Program budget management

 

Education Requirements:

·         Bachelor’s Degree / Master’s Degree preferred

Experience Requirements:

·         2 years experience in providing services to homeless /underserved populations, including 1-2 years of supervisory experience (preferred).
