Job Description:
The Transitional Case Manager works with hospitals, local jails, and correctional facility clients who have been identified as being mentally ill and in need of being connected to the Medication Assistance Program. The Transitional Case Manager coordinates connection to the local Department of Social Services to obtain housing and medication, assists with the application process and accompanies them to the intake appointment. The TM assists the client in following all discharge, parole, or probation plans. TM can also assist with other applications which might be beneficial to client.
Qualifications:
Experience working with individuals with an MH diagnosis.
Education:
Bachelor's degree in a Human Service or related field required
