FUNCTION:

The Life Coach provides direct services for people with disabilities in their home and community. Ensures each individual’s health, safety, and well-being by assisting him/her in acquiring, maintaining and improving self-help, socialization, and/or community living skills. Adheres and implements established goals and outcomes chosen by each individual and his/her support team. Reports to the Home of My Own Project Manager.

RESPONSIBILITIES:

1. Supports the achievement of individuals’ personal outcomes and goals.
2. Reviews and implements each individual’s plan of care. Provides for or assists each individual in performing routine self-help tasks, and developing social/community living skills.
3. Provides for or assists each individual in performing household tasks such as routine cleaning and laundry, grocery shopping, and nutritional meal preparation.
4. Provides for and assists each individual with tasks to maintain a healthy and safe living environment. Continually practices universal safety precautions regarding infectious disease.
5. Documents and maintains accurate records regarding each individual’s goals and daily activities.
6. Participates, either in person or via report, an annual, or as requested, support team meetings for an individual.
7. Maintains a professional and cooperative working relationship with individuals, families and other team members. Serves as a model of appropriate dress and behavior for the individuals supported.
8. Follow proper procedures to inform supervisors of matters relating to his/her employment including schedule changes and availability, hospitalizations, and on-the-job injuries.
9. Informs the Home of My Own Project Manager/ Home of My Own Supports Broker in a timely manner of matters relating to the health, safety, and welfare of an individual.
10. Implements established emergency procedures in the event of an emergency. Demonstrates and understands the responsibilities of reporting incidents per state and federal guidelines.
11. Complies with federal, state and county regulations as they apply to the care of individuals associated with A Home of My Own. Abides by all ACHIEVA and A Home of My Own policies and procedures.
12. Maintains annual mandatory staff qualifications. Completes 24 mandatory staff training hours each year.
13. Performs other related duties as assigned. Qualifications

The Life Coach Position requires the equivalent of a High school diploma with one-year related experience as a provider of direct care support for persons with disabilities. Satisfactory ACT 33/34 clearances required. Satisfactory FBI clearance required depending on individuals assigned. Valid Pennsylvania driver’s license and a properly insured vehicle may be required.
