About the team.

The Aurora Center for Life Skills (ACLS) works with clients who are dual diagnosed with a developmental disability and a co-occurring behavioral health issue.  Our mission is to stabilize clients for their mental health and support their ability to function in the community.  This is a multidisciplinary team and includes therapists, psychiatrists, nurse practitioners, case managers, and a peer specialist. We work collaboratively in our shared mission of supporting our clients in their recovery.

About the role.

The Case Manager will support clients in meeting their basic needs. The Case Manager provides comprehensive psychological and behavioral case management services within a continuum of care designed to improve or maintain clients’ functioning.

 

Responsibilities:

  • Serve as payee for clients who require assistance to manage their
  • Help clients in meeting basic needs for food, clothing, shelter, personal safety and general health care, and assist them with applications for income, medical, housing or other benefits which they may need and to which they are
  • Ensure clients' access to needed services and community resources by arranging for
  • Provide comprehensive psychosocial services designed to improve or maintain clients’ abilities to function
  • Help clients evaluate strengths and symptoms, and facilitate in clients setting their own goals and plan for appropriate
  • Guide and instruct clients in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, use of transportation and other community
  • Help clients develop social skills, interests and leisure time activities, including opportunities for age appropriate
  • Help clients find and make use of appropriate employment opportunities, vocational rehabilitation services or supported work environments where appropriate.
  • Offer support, assistance, consultation and education to families, friends, landlords, employers, community agencies and others who come into contact with clients, in order to maximize benefits and minimize problems associated with the presence of these persons in the
  • Identify, work with and make full utilization of potential natural support systems such as neighborhood networks, churches and community organizations to encourage treatment program engagement.

About you.

The ideal candidate should be a self-starter, have strong communication skills, capable of prioritizing demands and managing deadlines, and motivated to help others. They should have a familiarity with community resources and be able to assist clients with applications for various types of benefits/assistance. This teammate should be comfortable providing outreach, coordination and patient advocacy with various external agencies, including primary care providers. Previous experience working with the individuals experiencing behavioral health issues and prior case management or care coordination support is preferred. Requirements:

  • Bachelor’s degree in psychology, social work or other related human services degree. Or Bachelor's Degree in unrelated field with at least one year of behavioral health
  • Ability to assess crisis situations and intervene
  • Demonstrated interest in behavioral health field
  • Knowledge of community
  • Basic understanding and ability to work with differing behavioral health presentations.

  • Effective written and verbal communication
  • Preferred: Bilingual

Compensation, Benefits & Perks.

Salary for this role (based on experience and company equity):

  • $16.50 - $20.00 hourly

Additional Compensation Opportunities:

  • Language Differential Pay (For Bilingual Candidates): 5% of base salary up to $3,000/year
