Description

This is a full-time position (40 hours/week) Monday through Friday 8:30 am - 5 pm. This position is located at one of our Secure Residential Facilities in SE Portland.

The Counselor III provides clinical services to the secure programs.  The Counselor III will work as a member of an interdisciplinary team to implement and monitor treatment for residents within the secure residential facility, while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required the Counselor III understand the effects of trauma on health, coping, and other aspects of the lives of those we serve.  This position involves participation and implementation of program goals and objectives to promote a strengths-based, client centered approach to assisting residents in achieving individual goals and ultimately transitioning to a less restrictive setting. 

Essential Responsibilities

  • Provide therapeutic interventions and counseling with residents.
  • Integrate best practices into the day to day operations, including Trauma Informed Care and Motivational Interviewing.
  • Complete Behavioral Health Assessments, Treatment Plans, Safety Plans, and Behavioral Support Plans.
  • Becomes certified as a Commitment Investigator for Multnomah County and manages the commitment process for residents at Faulkner.
  • Provide group therapy, skills training and model group facilitation skills for other staff members. 
  • Generate therapeutic treatment plan. Provide treatment and manage caseload.
  • Provide individual therapy as directed by resident’s Treatment Plan; identify and prioritize with the client their treatment plan goals. Redefine goals as needed to obtain success; provide continuous assessment and measurement of progress toward treatment goals.  Consultation with the Program Manager, Program Supervisor and others is necessary in order to maintain treatment team communication. 
  • Coordinate with oncoming shift regarding resident concerns, building/program needs, and any issues relevant to ensure the smooth operation of the program.
  • Participate in regular supervision. Provide clinical consultation to the Primary Treatment Coordinator. 
  • Attend and participates in all meetings as assigned.
  • Coordinate treatment and continuity of care with community stakeholders, guardians, representatives from the legal system, hospital care, medical care, and other community based agencies in order to assist with the treatment needs of clients on case load.
  • Attend clinical case conferences and staff meetings to gain knowledge and communicate program and resident needs.
  • Comply with and implement the regulations, policies, and procedures under which the facility is licensed and operates.
  • Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations.  Maintain accurate and up-to-date documentation associated with billing, assessment, treatment, residents’ progress and other activities as required.
  • Provide input on all relevant documentation and procedural requirements.
  • Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.
  • Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for clients/residents and staff.  
  • Demonstrate responsibility for safety of residents, staff and property; possess familiarity with fire regulations and evacuation procedures.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Complete annual employee training requirements on a timely basis as defined by the agency and program.
  • Remain awake on all shifts.
  • Perform other duties as assigned.

Role Specific Responsibilities:

  • Take leadership on managing the process of re-commitment: create the appropriate legal documentation for the court, coordinate with the court system and civil commitment team at Multnomah County, and determine witnesses for the hearings.
  • Administer seclusion and restraint techniques as defined in non-violent physical intervention training.
  • Assist in crisis situations as appropriate, following emergency protocols and procedures.   Practice and demonstrate competency in avoidance, preventing escalation, and physical and mechanical restraint techniques (Class 1 Facility Only) as appropriate, to avoid crisis situations or to neutralize crises in progress.
  • Participate in a rotation to remain working on-site for up to eight (8) additional hours in the event that the following shift has inadequate staffing. 

Qualifications


Education:

A Master’s Degree in a behavioral science field from an accredited college or university. Demonstrate the competencies as outlined in the Oregon Administrative Rules (OAR) to become credentialed as a Qualified Mental Health Professional (QMHP). Certification must be current and in compliance with the OAR’s. The CADC credential is preferred in addition to a Master’s Degree given the high occurrence of substance use/abuse.

Experience:

A minimum of three years of clinical experience providing inpatient or outpatient services is preferred.

Specialized Knowledge, Skills and Abilities:

  • Ability to conduct a Mental Status assessment and formulate a diagnosis according to the most current DSM
  • Understanding of criminality and criminogenic risk factors
  • Understanding of recovery model, motivational interviewing, illness management and recovery, evidence based practices, and strength based case management philosophies and strategies
  • Proficiency in operating computer hardware and software (including EMR software)
  • Ability to enter and retrieve data; prepare simple reports

Other:

Possess a valid driver’s license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.

Must be able to perform CPR and First Aid.  CPR/ First Aid certifications are required and must remain current.

Physical demands:

Activities and structure support community integration assist program residents in learning socially acceptable behaviors. 

Employee must be approved to provide transportation utilizing agency vehicles so that residents may access the community.  Employee must also have the ability to assist residents in skill building outings and accessing community resources. The Counselor III must have the ability to assist the team in providing safety to all residents and staff in potentially volatile situations.  When the need arises, staff may be asked to perform non-violent physical interventions and holds.
