Under the supervision of the Deputy Chief of Police, the Social Services Coordinator is a member of the DeSoto C.A.R.E Team. As a part of a multidisciplinary team, this position will provide a combination of functions within the C.A.R.E Team office and the surrounding community, to include crisis prevention and follow-up, behavioral health screening and assessment, and referrals to community resources and providers. This position will be able to provide case management, individual and group psycho-education and psychosocial interventions, and complete person-centered recovery plans in an effort to support short-term stabilization of psychosocial and behavioral health needs. This position will be able to partner with law enforcement, the medical community, schools, families, and social services agencies in a collaborative effort to assist individuals in need. The individual must possess strong knowledge of the principles, methods, techniques and practices of working with individuals experiencing psychosocial and behavioral health needs within the community and have the ability to communicate effectively with diverse cultures and socio-economic populations. Other duties include documenting and maintaining information and files; contacting outside agencies; and facilitating cooperative efforts.
 
 


 

Essential Job Duties/Responsibilities:
Essential duties and functions may include the following.  Must possess required knowledge skills, abilities, and experience and to be able to explain and demonstrate, with or without reasonable accommodations that the essential functions of the job can be performed. Other related duties may be assigned. 

  • TYPICAL DUTIES:
      • Initiates contact with individuals identified by C.A.R.E Team staff that may need support, to assess and identify needs.
      • Fields community inquiries regarding services, how to initiate C.A.R.E team services, and referral and linkage to community services as appropriate.
      • Assists individuals experiencing psychosocial or behavioral health needs by conducting interviews and needs assessments; providing education and intervention services, providing information on available community services; contacting outside agencies; scheduling appointments; developing recommendations for referrals; and utilizing community resources to aid in recovery planning.
      • Develops plans of action by interviewing individuals; contacting agencies and setting appointments; explaining court processes; providing referrals and follow up; and ensuring delivery of services.
      • Maintains documents and information by preparing summaries of cases; documenting referrals and needs; maintaining notes on all interactions; and filing reports and documents.
      • Ensures service delivery by interacting and working with cooperating community partnerships and agencies.
      • Advocates effectively within the service delivery system to facilitate needs services for individuals and to protect rights of those served.

      KNOWLEDGE, SKILLS, & ABILITIES:
      • Ability to effectively communicate, both verbally and in writing, including sharing information, sharing ideas with and giving positive feedback to supervisor, co-workers, the public, and elected and appointed officials
      • Skill in the operation and use of standard office software and equipment which includes databases and presentation programs, along with standard Microsoft products
      • Ability to organize and prioritize work assignments
      • This position requires a flexible workday and a person who is able to work both independently and collaboratively amongst a team

     
EDUCATION & EXPERIENCE:

Minimum Qualifications:
  • Master's Degree from an accredited college or university in social work, psychology, or counseling
  • Licensed mental health provider. 
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) preferred; LMSW and LPC-A will be considered with experience
  • Two years of work-related case management experience 
SUPPLEMENTAL INFORMATION:

Licenses and Certifications:
  • Valid driver's license with good driving record and ability to maintain while employed.
  • Licensed mental health provider. Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) preferred; LMSW and LPC-A will be considered with experience
 
 WORKING CONDITIONS:
 
Position requires mobility within both an office environment and the community, and requires little physical effort in working with light materials generally weighing ten (10) pounds or less.
