PURPOSE: The Community Representative will positively represent Grace Healthcare Services with the intent to develop early and appropriate referral source relationships by creating and sustaining business partnerships with new and existing referral sources.
QUALIFICATIONS:
Bachelor’s degree or professional license preferred
Minimum two (2) years Outside field sales/marketing/provider relations experience preferred
Minimum two (2) years healthcare experience preferred
Understanding of healthcare industry including physician relationships and/or a demonstrated capacity to learn clinical knowledge
Excellent communication skills
Ability to commute within the state of NJ
RESPONSIBILITIES (including, but not limited to):
This position does not have supervisory duties
Develop business partnerships by making effective sales contacts and presentations/in-services.
Initiate and maintain contact using a mix of communications tools.
Develop, implement, and evaluate quarterly and annual territory plans to achieve monthly admissions goals. Implement sales strategies and actions.
Identify and develop key referral sources within the community through a data-driven targeting process for earning referrals.
Use key referral sources to grow the census reaching and exceeding monthly goals.
Re-establish and repair strained relationships with facilities and doctor offices as needed.
Promote hospice services to all healthcare providers.
Meet with families and patients to explain the benefits of end of life care.
Obtain and maintain required contracts including yearly contract compliance requirements.
Take the initiative in building relationships within all internal and external departments to assure establishment of effective communication with referral sources and internal customers.
Update and maintain customer and competitor profiles, log daily call activity including any open requests and the follow-up plan for future customer interaction.
Maintain and increase knowledge of Hospice services, customers and referral sources.
Field Activities accounts for 80% of the position: Participate in presentations, exhibits and community events as requested. This may include overnight stays.
Work with the Business Development Manager and/or marketing team to develop targeted messages and marketing tools for the referral audience.
Plan and participate in educational programs and in-services to various healthcare and long-term care groups, physician offices, hospitals and the community.
Coordinate communications between physicians, discharge planners, case managers, medical staff and caregivers regarding the admission and discharge of patients.
Serve as an educational resource to the community regarding hospice care.
Ensure monthly IDT meetings are scheduled and sin-in sheets are collected as appropriate.
Continue career development through self-study and one-on-ones with the hospice medical director and nurse practitioner.
Maintain professional and clinical competence.
Maintain positive, professional working relationships with all Team members.
Market hospice services to physicians (Oncology, IM, Nephrology, Cardiac, Pulmonology, Hospitalists, Geriatric, and Family), facility staff, and other health care professionals as well as hospitals, long term care facilities, and assisted living and skilled nursing homes.
Establish and Adhere to the practice of confidentiality with regard to patients, staff, customers and all aspects of the organization.
Ensure that Grace Healthcare Services best practices are incorporated into all job activities/responsibilities
Perform other duties as necessary
