In 2017, the San Francisco Department of Disability and Aging Services (DAS) determined that Institute on Aging (IOA) would implement the Support at Home Program (S@H). The goal of S@H is to serve eligible adults with disabilities and older adults living in the community by providing resources for needed home care that are not available through any other mechanisms due to affordability challenges. PRIMARY RESPONSIBILITIES: Conducts functional and financial needs assessments of adults with disabilities and older adults who have found it difficult to afford home care services to allow them to continue living independently in their homes, or to return to community living from institutional placement. Maintains monthly telephone contact with clients and/or third parties to monitor ongoing home care services, safety, and satisfaction, including psychosocial, health including mental health, situational, and environmental needs. Facilitates home care coordination and advocacy for operations and services to meet a client’s home care needs and promote stable community living and the values of dignity and aging in place.
