Description
Integrated Life Choices is seeking a Case Manager for individuals with developmental and intellectual disabilities in our services. We are looking for a dedicated individual to join our team in the Denver area. This position will be responsible for overseeing the integrity of all medical related aspects of the assigned ILC caseload of individuals supported.
Duties and Responsibilities include:
• Schedules and tracks medical appointments.
• Enters and adjusts medications for established individuals that ILC supports.
• Reviews all individual’s incoming mail.
• Maintains appointments, inputs caseloads, responds to scomms, and other maintenance as needed in Therap.
• Acts as liaison between Physician offices, pharmacies, and nurses.
• Maintains all confidential records.
• Routes all Physician contact forms for individuals ILC supports per ILC policies.
• Routes all GERs for individuals that ILC supports.
• Serves on Quality Improvement and Therap Committees.
• Creates annual PMIPs and ensures they are submitted to Medicaid LTC.
• Conduct Medicaid revalidations, tracking when they are due, completing the report, and submitting.
• QI review of Med and program books at HHPs and Voc offices; ensures medications are being stored and documented correctly.
• Conducts medication and confidential book reviews for all individuals ILC supports.
• Sets-up new individuals with books, medications, etc.
Requirements
Minimum of 19 years of age. Valid Driver’s License, Medication Aide, Associate's degree (A. A. / A. S.) or equivalent from two-Year College or technical school; and two years related experience and/or training; or equivalent combination of education and experience is preferred. Experience working with Therap a plus.
Successfully complete and pass all checks including:
• CAPS Background
• Criminal Background Check
• DMV
