Position Summary: The Licensed Master's level Social Worker, reporting to the department manager, works in collaboration with the health care team and is responsible for assisting the Treatment team in the development and implementation of patient discharge plan of care. Licensed Social worker will assist the team with assessment and analysis of patient's treatment plan. Participate in Multidisciplinary care of Geriatric Psychiatric patients; Facilitate the referral and transfer of patents to the appropriate community services i.e. home care, skilled care, acute rehab, assisted livings, nursing homes, etc. Act as liaison between patient, hospital and community services. Communicate frequently to patients and families on the discharge plan of care and lead Treatment team meetings. Patients require an initial three day Treatment team meeting and then a seven day after which it continues with seven day Treatment team meetings thereafter. Licensed Social Worker will also provide support to the Geriatric Psychiatric Unit by performing psychosocial assessments and develop appropriate discharge plans on identified patients by provide community referrals and resources for families as needed. Group treatment activities will be performed meeting the need level of the patients at least weekly.
Essential Job functions:
Implement discharge plan based on identified needs and barriers from Acute Psychiatric Case Management Assessment.
Communicate with multidisciplinary team and patient/family the plan for discharge and the anticipated services needed to complete transitions to the next level of care.
Collaborates with multidisciplinary team to manage unit specific LOS (length of stay). Assures LOS is at target.
Provide support to the Geriatric Psychiatric unit and patients.
Take inquires for the unit and work with NP to obtain admission permission and work with Insurance companies for pre-authorizations when needed.
Assures effective transition of patient to next level of care through communication of discharge needs and plan to providers at next level of care.
Assures compliance to regulatory standards/requirements ie. Important Message from Medicare Letter, Home Care Face to Face form; Medicaid Level of Care applications.
Assists in the timely initiation of Legal Guardianship appointment and or Medicare or Medicaid Applications through the various county agencies, as needed.
Participates in Team Development Opportunities.
Actively exhibits both independent and departmental accountability and takes actions to drive change (exhibits qualities of a culture of "I saw it, I own it, and I'll help to provide a solution).
Other duties as assigned.
Education and Experience: Master's degree required. Must be a licensed Social Worker in the state of Ohio. LISW preferred. Three to five years of Social Work experience in geriatrics and or acute psychiatric care setting.
