Position Overview: The Adult Family Care (AFC) RN works collaboratively with the AFC Care Manager to conduct initial and on-going in-home assessments of consumers, caregivers, and the AFC home to determine eligibility for the program. The AFC RN, in conjunction with the AFC CM, is responsible for developing the AFC care plan, supporting, training caregivers, and ongoing maintenance of the AFC program for the consumer and caregiver.
Responsibilities include: Completes assessments of potential long-term clients within two weeks of date of referral to determine eligibility. As a member of the AFC team, recommends suitability of clients for Adult Family Care. As a team member, participates in the recommendation of client/caregiver matching. Once a potential caregiver is identified, participates in initial match visits as needed. Initiates and develops client’s medical portion of plan of care within one week of placement and submits them to client’s physician for recommendation and approval. In conjunction with the case worker, evaluates caregivers and makes home assessments to determine the suitability of the home and the caregiver’s abilities. Presents completed assessment to the team. Performs the initial caregiver training related to the identified client and respite teaching as needed. Advises caregivers and clients of community health resources (i.e.-ADH programs). Refers, coordinates and monitors the services they receive. Monitors caregivers to make certain they arrange routine medical appointments and yearly physicals for clients. Participates in program outreach, including presentations, information dissemination or other activities as directed.
Qualifications: The ideal candidate will have excellent:
Current Massachusetts RN license. Must have a valid driver’s license, reliable transportation, and be willing to travel throughout the service area.

This is Monday-Friday, 35 hour-per-week position, with no nights and no weekends!
