Position Overview: An ASAP RN assess clients for services, determine clinical eligibility for programs and develop care plans. Our primary goal is to support individuals who receive services in the community so they can continue living in their own home. Duties include data entry into computer database, communication and consultation with family members, other Montachusett Home Care staff and community professionals.

Responsibilities include: Participates and oversees the Personal Care (PC) Service Program according to EOEA guidelines.  The RN determines consumer eligibility for personal care service, develops the personal care plan, and reassesses as needed.  Completes screenings for MassHealth recipients or applicants seeking to: attend Adult Day Health programs, or become eligible for the Frail Elder Waiver, or to enter into a skilled nursing facility.  The RN determines continued nursing home eligibility for MassHealth consumers/applicants presently in area skilled nursing facilities, and assists with discharges from area skilled nursing facilities.  Provides consultation and in-service training to staff, provider agencies and other community agencies.

Current Massachusetts RN license with experience in gerontological nursing or relevant nursing field is required.  Must have a valid driver’s license, reliable transportation, and be willing to travel throughout the service area.

Compensation and Benefits:

This is Monday-Friday, 35 hour-per-week position, with no nights and no weekends!  We also offer competitive pay, mileage reimbursement, excellent work environment and an outstanding benefits package, including health, dental, vision, company paid life insurance, short term disability, and health reimbursement account.  Additionally, this position offers a 403B retirement plan, flex scheduling/remote options, generous paid time off, and employee referral bonuses!
