About San Francisco Campus for Jewish Living

The nonprofit San Francisco Campus for Jewish Living serves a significant number of patients and long-term care/skilled-nursing residents each year in its Jewish Home & Rehab Center. Non-residential care is offered through our short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through our acute geriatric psychiatry hospital. With over 650 compassionate, experienced clinical and allied health personnel, SFCJL provides older adults with a variety of life-enriching healthcare programs and services in a welcoming, stimulating environment.

SFCJL’s campus includes the new Lynne & Roy M. Frank Residences (assisted living apartments and memory care & support suites), and the membership-based Byer Square, a non-residential community of programs, services, and support.

Position Overview:

The Admissions Community Liaison is responsible for supporting admissions to the Jewish Home & Rehab Center. The position works closely with acute-care hospitals and community stakeholders to ensure a safe transition between care settings for new residents, acting as a liaison between the Admissions team, the community we serve, and the Home’s Interdisciplinary teams.

The Admissions Community Liaison will work closely with the Director of Admissions, playing an integral role in helping to achieve the organization’s census goals, growth, and revenue objectives.

 

Essential Functions:

  • Assist in the management of census and patient/resident ‘throughput’ with a focus on meeting/exceeding stated census goals and targets.
  • support the management of application throughput and data entry via different programs/modalities (EMR, Allscripts, Curaspan, Salesforce, Fax, Phone, Email, and LTC roster).
  • Monitor and provide on-going phone, email, and customer service support to the Admissions office for both Short-Term Care and Long-Term Care inquiries. Provide tours of units. Respond to inquiries and applicants in a timely manner.
  • Assess and review Long-Term Care applications for completion and appropriateness for potential admission. Communicate with applicants for follow-up until the application is complete and determination for admission and/ or admission has been finalized.
  • Perform room check to ensure the room is prepared for admission & order DME as needed.
  • Work with Clinical Liaisons to identify and follow-up on clinical needs/requirements for admission and communicate to the Interdisciplinary team as appropriate.
  • Create and complete admissions paperwork for incoming admissions, input incoming admissions into EMR, take the patient photograph, upload photo & appropriate documents to EMR.
  • Look-up, review, and verify insurance benefits/eligibility for potential patients/residents. Obtain authorizations as needed. Demonstrate understanding of contracted insurance providers, correctly input payer information into EMR, verify and save verification documents into Admissions drive.
  • Establish and maintain significant professional relationships with referral sources, physicians, and other healthcare professionals to increase referrals.
  • Represent JHRC in a professional manner in the community. Demonstrate patience, positive disposition, and enthusiasm with experience in customer service – “forward-facing” expectations, roles, and responsibilities.
  • Attend staff meetings and educational in-services as scheduled.
  • Perform other duties as assigned (as they relate to the Admissions department). This includes cross-training and coverage for other positions within the department.

Qualifications

  • High school diploma or equivalent, required. AA/AS/BS/BA preferred.
  • Demonstrated proficiency with MS Office, including Outlook, Word, and Excel, required.
  • Demonstrated experience working in a fast-paced environment, required.
  • Ability to multi-task with a sense of urgency while maintaining a sense of sensitivity, required.
  • Demonstrated a high level of interpersonal skills to handle sensitive and confidential situations.
  • Demonstrated ability to develop and build strong, lasting relationships with referral sources, clients, colleagues, and families.
  • Excellent organization, customer service, and communications skills, both written and verbal.
