About:

The nonprofit San Francisco Campus for Jewish Living serves a significant number of patients and long-term care/skilled nursing residents each year in its Jewish Home & Rehab Center. Non-residential care is offered through our short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through our acute geriatric psychiatry hospital. With a complement of close to 650 compassionate, experienced clinical and allied health personnel, SFCJL provides older adults with a variety of life-enriching healthcare programs and services in a welcoming, stimulating environment.

SFCJL’s transformed campus includes the new Lynne & Roy M. Frank Residences (assisted living apartments and memory care & support assisted living suites), and Byer Square, a non-residential community of programs, services, and support.

Position Overview:

Under the supervision of licensed nursing personnel, performs direct resident/patient care duties and assists with promoting a comfortable physical and psychosocial environment for resident/patients. Duties are performed in accordance with established policies and procedures and in a manner, which respects the uniqueness, dignity, need for self-respect and self-determination of the individual, all in ways, which promote maximum personal and functional independence.

Essential Functions:

  • Assisting with the unit-based meal service. e.g. plating meals and serving food and beverages. Assisting residents to and from dining areas in accordance with meal service schedules.
  • Clean resident/patient equipment and personal care/grooming items per established procedure.
  • Assists in ensuring that resident’s/patient’s closets, bedside stands, drawers, are clean, orderly, and odor free.
  • Ensures that resident/patient rights are maintained and respected, which includes confidentiality
  • Observes and reports any suspected changes in resident’s/patient’s functional and cognitive behavior to the Unit Manager or Charge Nurse.
  • Assists with the admission, transfer and discharge of residents/patients. Specific tasks include but are not limited to assisting with unpacking/packing and organizing belongings, acclimating the resident/patient to the unit, as well as provide helpful information about the facility.
  • In collaboration with the Life Enrichment Coordinator, encourages and assists residents to participate in the activity program. Also, assist the Life Enrichment Coordinator with activities as time allows.
  • Assists Environmental Services to maintain a clean, safe, homelike environment for residents/patients and staff. Reports unsafe or faulty equipment
  • Monitor and replenish resident/patient care articles. Also, transport supplies to and from the unit, as well as assisting in ensuring care equipment and supplies are used and stored in safely and efficiently.
  • Transport residents/patients to meals, activities, care conferences, appointments, etc.
  • Ensures residents personal items are marked, handled, and stored properly. Processing of soiled personal clothing as per policy and procedures.
  • Answers call bells promptly and provide non-caregiving tasks (i.e. change TV channel, open windows, bring items within reach, etc.).
  • Relay requests for actual care assistance to appropriate nursing staff.
  • Actively participates in facility programs, meetings, committees and educational in-services.
  • Encourages and adheres to a holistic philosophy of wellness, recognizing that each individual is capable of, to varying degrees; making decisions about his/her life and is a partner in the decision- making.
  • Actively participate in ongoing Quality Assurance and Performance Improvement tasks, initiatives, projects, etc.
  • Participates in collecting data needed for the accurate completion of the MDS and the plan of care.
  • Float and assist on other units as needed.
  • Performs other related duties as

 

Qualifications:

  • High school diploma or equivalent
  • Must be capable of maintaining regular attendance
  • Demonstrated high level of interpersonal skills to handle sensitive and confidential situations
  • Demonstrated ability to build rapport with peers and customers
  • Must have the ability to relate professionally and positively and work cooperatively with residents/patients, families, and other employees at all levels
  • Ability to read, write, communicate and comprehend verbal and written instructions in English
