Overview

Clinical Social Worker is a core member of Sanitas Bewell program. The Clinical Social Worker is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

Responsibilities

• Support the health care patient’s needs on an assigned patient caseload. • Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.• Screen and assess patients for common mental health and substance abuse disorders. • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.• Provide patient education about common mental health disorders and theavailable treatment options.• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.• Participate in regularly scheduled (usually weekly) caseload consultations.• Provide individual and group brief behavioral interventions using evidence-based techniques such as behavioralactivation, problem-solving treatment, motivational interviewing, or other treatments asappropriate.• Builds relationships with community organizations and serves as the point person connecting patients and community resources.

Qualifications

• Master’s degree in related field• BLS certification • Compassionate and caring demeanor• Ability to build rapport with clients• Strong leadership qualities• Excellent written and verbal communication skills• Bilingual Spanish and English
