Experience:

-  A minimum of five (5) years direct leadership in the Healthcare or related arena required.

Prior experience with facilitating community healthcare access required.

-  Prior experience with state, community and health system programs supporting health and wellness, Access Health, baby friendly, Transitional Care Management (TCM), Chronic Condition Management (CCM), and Diabetes Prevention Program (DPP) preferred.

-  A minimum of five (5) years of direct experience with supervising/managing clinical healthcare programs and staff required.

-  Prior experience with local community interactions and familiarity with the regional healthcare environment, physician network, human service organizations another related entities preferred.

 

Education:

-   Bachelor degree required. Master degree preferred.

-  Education in social work, nursing, or a public health/Healthcare related area preferred.

 

Special Skills:

- Experience with clinical programs including: Access Health, Transitional Care Management (TCM), Healthy Outcomes Program (HOP), Baby Friendly Hospital/Practices, Care Coordination and RN Clinical Navigation preferred.

Ability to effectively manage and lead complex operations required.

-  Experience with budget preparation and management required.

-  Exemplary communications skills in both written and non-verbal format required.

-  Ability to function in demanding and stressful high paced work environment required.

-  Exemplary critical thinking and problem solving skills required.

- Proficient with Microsoft Office software programs such as Excel. Word, PowerPoint and Outlook required.

Position Summary:

     The Assistant Director of Tidelands Community Health Resource Center is responsible for assisting with guidance and oversight of all Community Health Resource department programs/services.  This includes assisting the director or directly providing oversight of operations in the director’s absence for the Community Wellness programs, Transitional Care, Expectant Moms and Newborns, TCCN, and Community Palliative Care. This also includes guiding teams in strategic planning, implementation and operations of those services.

     The Assistant Director of Community Health Resources is directly responsible for the clinical programming in the department.  This includes providing both guidance and oversight of (1)Tidelands Community Care Network (TCCN) which is an access health program focusing on access to care for the uninsured; (2) Transitional Care Management (TCM) which is a Medicare reimbursable care coordination program; (3) OB Education/Lactation, including the Baby Friendly initiatives by the hospital system; (4) Personal Health Navigation Services for employee partners and industries under contract; (5) Community Palliative Care.

 

Responsibilities include:

  • Assisting to convene, facilitate and foster community partnerships in support of identified community health needs and Tidelands Health Strategic initiatives
  • Ensuring a multi-disciplinary approach to person centered care with a blending of social determinants of health and chronic condition management with the goal of increasing self-management.
  • Facilitating and fostering provider relationships to grow access to primary and specialty care.
  • Ensuring a presence and voice in the hospital system, particularly in topics associated with social determinants of health, health disparities, access to care and Triple Aim
  • Ensure wholistic patient centered care is provided in all realms of clinical work, with special focus on TCCN and TCM participants/patients and grant metrics and goals.
  • Ensuring Baby Friendly requirements and certifications are completed and monitored for the system.
  • Ensuring DPP and Diabetes initiatives related to Community Health Resources are completed including but not limited to: completion of DAP requirements, CDC recertifications, A1C labs and DPP class fidelity and scheduling with strong coordination and involvement with the Diabetes Center and providers.
  • Ensure that care coordination and clinical navigation efforts include comprehensive care pathways and care plans that address: chronic health conditions, social determinants, and palliative care services.
  • Ensuring compliance with grants and other funding sources; being a resource and initiator of grants that fit with the strategic initiatives of Tidelands Health.
  • Responsible for grant management/billing integrity/financial management and oversight for areas under clinical programming, including but not limited to: Access Health grant, TCM documentation, Baby Friendly/ENRICH, ACP Grant, DPP/Diabetes Project initiatives, etc.
  • Overseeing the human resource and fiscal operations of the clinical programs in the department.
  • Ensuring compliance with hospital and departmental policies.
  • All other duties as assigned.
