Hourly Wage for WELLLIFE NETWORK Case Manager [MH-ACTW8] Salary in the United States

POSITION SUMMARY: The Case Manager will provide supportive services to team members in order to assist them with the needs of recipients within the ACT team. The Case Manager will perform a range of tasks which are designed to assist the team with completion of various applications as well as health home and CAIRS data entry.

ESSENTIAL ACCOUNTABILITIES:
1. Primary responsibility for inputs into GSI for caseload including monthly contact note, HML, plans of care, and other tasks TBD
2. Primary responsibility for input into CAIRS upon admission, annually (bi-annually for AOT), and discharge
3. Primary responsibility for planning, organizing, and attending monthly socialization/wellness group.
4. Primary responsibility for all basic needs shopping trips and completing any applicable service dollar request forms
5. Complete SPA applications (with assistance from primary clinician for clinical information)
6. Complete SPOA applications (with assistance from primary clinician for clinical information)
7. Complete SCAT applications (with assistance from primary clinician for clinical information)
8. Representative payee application
9. Send social security redetermination paperwork
10. Sending release and paperwork to DSS
11. Safelink/Assurance cell phone applications
12. Medicaid utilization threshold override applications
13. Attend and participate in at least 2 team meetings per week
14. Coordinate with primary clinician and primary nurse as needed between team meetings
15. Participate in supervision with Team Leader at least quarterly.
16. Document any progress notes in EHR within 48 hours of all client and collateral contacts, as well as for status updates on any application processes.
17. Participate in OMH approved ACT Orientations and Trainings.
18. Ability to operate an agency vehicle.
19. Participate in activities of other staff members in their absence or during periods of staff shortage.
20. Maintain confidentiality at all times.
21. Responsible for reporting and documenting all incidents to immediate supervisor within 24 hours.
