Job Description Job Description HSAO’s JPT program is an empirically based process that empowers the most high-risk youth and families in Allegheny County by building a cohesive team of professionals and natural supports. This team uses monthly meetings to design a personalized plan for families to reach their vision for a better future. The Youth Support Partner is a full-time position that works with youth throughout Allegheny County who have agreed to participate in the Joint Planning Team Process. The Youth Support Partner will work as part of the Joint Planning Team Workforce, which includes Coach, Facilitator, and Family Support Partner. Youth Support Partner provide support and guidance to the youth throughout the JPT process, during meetings, and in completion of the individualized plans developed by the youth and their family. The Youth Support Partner will work with up to 25 youth to develop and effectively utilize their own team of supports (natural, community, and systems).
