Provides comprehensive case management services for assigned clients including but not limited to advocacy, assistance in problem solving, assessment, conflict resolution, entitlements, training on daily living skills, and referrals.

Collaborate with Social Worker as well as other health care providers to share assessment and treatment plan data to assist in the facilitation of a smooth transition both to and from the hospital setting as appropriate including out-of-state and country providers.
