Assist clients with transition into supported housing apartments in Pinellas Hope, preparing a service plan with them, including healthcare needs, budgeting, monitoring cleanliness and needs for maintaining housing stability in PH V, addressing self-assessment need areas, and act as CM support as needed to assist client in maintaining stability while promoting self-sufficiency.
