Job Overview

Bradford City Primary Care Network 4 is looking to recruit a forward thinking and enthusiastic experienced Social Prescribing Link Worker.

We are seeking to appoint a flexible minded Social Prescriber to take a front line role working with the practices, supporting patients to the best possible care and support. This role will help empower local residents to take control of their health and wellbeing, allowing them to create time to focus on what matters to the patient.

The Social Prescriber supports taking a holistic approach to an individual’s health and wellbeing. The role will involve connecting people to community groups and statutory services for practical and emotional support.

The Social Prescriber will provide support for low to medium intensity patients with mental and social health conditions to overcome barriers. The Social Prescriber will be embedded within Primary Care Network multi-disciplinary teams to provide personalised support to individuals, to enable them to take control of their wellbeing and improve their health outcomes.

The successful candidates will have excellent communication skills and a high level of IT literacy.

Main Duties of the Job

The role will help people manage their health differently and more effectively, the aim is to help people who are lonely or isolated, or who have complex social needs which affect their wellbeing, including their mental health.

You should be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges.

The successful candidate should expect to work across all the practices delivering care to people from a wide range of backgrounds. At the centre of your workload and commitment to patients is to work in a timely, efficient and confidential manner at all times.

About Us

Bradford City Primary Care Network 4 (PCN4) is a network of Bradford City practices working collaboratively across the network area to look after the health of our local population. The PCN which comprises of 5 local practices with a total population of around 42,000 registered patients.

Job Description

Job responsibilities

· Take referrals from the PCN’s Core Network Practices

· Support the health and wellbeing of patients

· Assess how far a patient’s health and wellbeing needs can be met by services and other opportunities available in the community

· Co-produce a simple personalised care and support plan to address the patient’s health and wellbeing needs by introducing or reconnecting people to community groups and statutory services, signposting where appropriate

· Evaluate how far the actions in the care and support plan are meeting the patient’s health and wellbeing needs

· Provide personalised support to patients, their families and carers to take control of their health and wellbeing, live independently, improve their health outcomes and maintain a healthy lifestyle

· Develop trusting relationships by giving people time and focus on ‘what matters to them’

· Take a holistic approach, based on the patient’s priorities and the wider determinants of health

· Work in partnership with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care

· Work proactively to develop relationships with external providers to facilitate joint case management of patients accessing multiple services

· Educate non-clinical and clinical staff within the PCN on what other services are available within the community and how and when patients can access them. This may include verbal or written advice and guidance

· Manage and prioritise their own caseload, in accordance with the health and wellbeing needs of their population

· Where required and as appropriate, refer patients back to other health professionals within the PCN.

· Knowledge and experienced in using SystmOne

· Excellent working knowledge of Microsoft Office applications

· Can demonstrate personal accountability, emotional resilience and ability to work well under pressure

· Shows a flexible attitude to team roles and work effectively within a team to meet shared objectives

Person Specification

Essential

· Meet DBS standards

· Willingness to work flexible hours

· Able to travel across multiple sites/locations

· Ability to use Microsoft office and word

· Understanding of safeguarding policies for children and vulnerable adults

· Ability to recognise and work within limits of competence and seek advice when needed

· Experience of working in health and social care and other support roles in direct contact with people

· Ability to travel across locality on regular basis

· Own transport

· High level of written and verbal communications skills

· Knowledge of and ability to work to policies and procedures

Desirable

· Experience of working with elderly or vulnerable people

· Experience of working directly in a care coordinator role, adult health and social care

Qualifications

Essential

· NVQ Level 3, Advanced level or equivalent qualifications or working towards

· Demonstrable commitment to professional and personal development

Desirable

· Motivational Interviewing techniques
