Location: Basingstoke
We are looking to recruit a Practice Lead Nurse at CGH Partnership.
Key Responsibilities:
To work collaboratively with the primary care team to meet the needs of patients with support from the senior nurse manager
- To lead nursing and health care team
- To overview and supervise the nursing team, including audits, rota, practice policies, infection control, and overall performance
- To deliver individual treatment plans for patients within clinical competency.
- To deliver general nursing services such as Wound Care, Cervical Cytology, Health Screening, Vaccination and Immunisation Programme, Diabetes Management, Travel Vaccinations, Chronic Disease Review, Dementia Management, Cancer review, Asthma Management.
- To participate in regular practice meetings with the multi-disciplinary team.
Leadership Responsibilities:
Shared management of nursing team
- Support the practice to achieve its requirements within the GMS contract, QOF framework, enhanced services and other policies and protocols.
- Ensuring CQC compliance standards for the nursing team are met and maintained
- To provide nurse mentorship and supervision as required
- Act as a positive role model for nurses through the demonstration of strong clinical knowledge.

To be considered for this exciting role you must have:
- Candidate must be NMC registered
- Evidence of continuing professional development
- Demonstrates sound evidence of academic links
- Ability to priortise and manage own workload
- Ability to work as part of a team or on own initiative
- Experience in Primary Care Health settings
- Understanding of the Lead Nurse Role
- Effective management and organisational skills
- Clear thinking and problem solving and decision-making skills
