Job Description
Are you an experienced social worker with a commitment to supporting young people leaving care?
We are seeking a Social Worker to join the Pathways Leaving Care Team in Middlesbrough. In this role, you will work closely with care leavers, helping them to navigate the challenges of independence and achieve their full potential.
Responsibilities
- Managing a caseload of young people leaving care, ensuring their needs are assessed and addressed.
- Developing and implementing Pathway Plans that promote independence, well-being, and positive outcomes.
- Supporting care leavers in accessing education, employment, housing, and other essential services.
- Collaborating with multi-disciplinary teams, external agencies, and other professionals to coordinate services and resources.
- Ensuring compliance with relevant legislation, policies, and procedures related to leaving care and child protection.
Qualifications
- Qualified Social Worker registered with Social Work England.
- Significant experience in a social work role, particularly working with care leavers or young people in care.
- Excellent communication and interpersonal skills, with the ability to build effective relationships with young people, families, and professionals.
- Knowledge of relevant legislation, policies, and procedures related to leaving care and child protection.
