Description:
The PAN Nurse is responsible for providing direct nursing care to the residents. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility
Responsibilities:
- Provides clinical operational oversight, education, and support for the post-acute skilled nursing programs (cardiac, respiratory, neurology and wound care)
- Provides clinical expertise, problem solving, and critical thinking.
- Monitor the effectiveness of the program as well as to assist with the implementation of identified processes that improve the transition of patients across the continuum of post-acute services.
- Resource for clinical leadership teams at the facility.
- Manages and makes recommendations for the operational processes that insure cost effective care.
- Serves as the clinical liaison between the post-acute network (PAN) and skilled nursing facility.
Qualifications:
- RN, preferably BSN.
- Minimum 2 years clinical experience in medical-surgical nursing and post-Acute skilled nursing. Competent in IV therapy, antibiotic management, medication management, laboratory, and diagnostic tests.
- Thorough understanding of Medicare, Medicaid, Medicare Advantage, and private insurance.
