Physical Therapist Responsibilities:
- Conduct assessments of referred patients.
- Deliver direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation.
- Develop and implement treatment programs with tailored goals, guided by the patient's capacity and under physician direction.
- Set realistic patient goals, maintain comprehensive documentation, and educate patients and families on care plans.
- Collaborate with all disciplines for optimal patient-centered treatment and care.
Physical Therapist Qualifications:
- Graduation from an accredited Physical Therapy college or university.
- Current and unencumbered Physical Therapy license for the State of NY.
- Experience in long-term care and/or geriatric care preferred.
- Proficiency in EMR documentation (specifically Wep PT).
- CPR certification (Preferred).
- Bilingual in English/Spanish.
