Position Overview:
This position, in cooperation with Primary Care Physician, hospitalist and hospital staff, facilitates the Hospice admission process. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, including level of care, Durable Medical Equipment (DME), comfort medications and transportation. This position enters referral into Electronic Medical Record and completes referral process and admits patient as appropriate. This position forwards hospital documents to Hospice and communicates with Hospice and Hospital Staff. This position provides education to hospital staff and physicians. This position is able to make home hospice visits to provide patient care coverage as the business need requires.

EDUCATION:
Graduate from an accredited school of nursing with a current California Registered Nursing license.
CERTIFICATION & LICENSURE
RN-Registered Nurse of California
BLS-Basic Life Support Healthcare Provider
California Drivers License
Automobile Insurance
PREFFER EXPERIENCE AS TYPICALLY ACQUIRED IN
5 years hospice experience required.
5 years direct patient care experience
SKILLS AND KNOWLEDGE
Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice and palliative care for terminally ill patient. Should also possess knowledge of hospice reimbursement methods. Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care. Ability to recognize the special needs of Hospice patients and others in the home. Ability to communicate with patients and significant others in an effective, mature, caring manner. Ability to maintain harmonious constructive relationships with internal and external customers. Demonstrated effective written communication skills. Ability to be aware of surroundings and respond to emergency and/or urgent events. Ability to select and utilize patient transfer/transport equipment safely. Productivity standards - ability to meet productivity expectations. Must be detail-oriented.Ability to work in mentally and physically demanding environment.
Pay Range: 66.61-88.58
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
