Division Overview: The Adult Criminal Justice and Treatment Services (ACJS) supports clients who may have struggles related to a substance use disorder. We provide opportunities for clients to rebuild their lives so they can become healthy and self-sufficient. ACJS’ work includes screening and assessments for substance use disorders, placing clients into treatment programs, monitoring and reporting on progress, and providing support including, but not limited
to, insurance enrollment, obtaining a state ID, and finding employment resources.
We are currently looking for full-time Care Navigator:
Position Summary: This position is responsible for the care of participants in the SUPR/SORS program in collaboration with Miles Square Health Center to provide coordinated care for justice-involved adults with stimulant or opioid use disorder. Partner with Miles Square Health Center to provide identified clients with comprehensive medical care in addition to any Medication Assisted Recovery or other evidence-based programs for their identified substance use disorder. Provide referrals/linkages to clients for identified needs and coordinate outreach services to community based social service agencies. Work with clients to remove barriers to care including insurance, transportation, child care, etc. Work as part of an interdisciplinary team including both TASC and Miles Square Health Center staff.
ESSENTIAL DUITES AND RESPONSIBILITIES:
- Conduct outreach to engage clients into health services provided by Miles Square Health Center, schedule intake appointments in conjunction with MSHC intake staff including addressing transportation needs
- Coordinate and implement the delivery of consistent and quality program services
- Maintain program information and technology affecting functional area(s) to increase program effectiveness and ensure compliance
- Prepare, interpret, and present reports to the Coordinator, external funding sources, and community partners in accordance with established timeframes
- Serves as direct liaison to the Coordinator for program related needs and operational issues
Requirements
Our Ideal candidate:
- Bachelor's degree preferred in healthcare, human services or related field from an accredited college or university with at least three years of experience in a related field.
- Must be able to function independently and ability to work effectively with individuals, and support agencies and stakeholders
- Ability to manage from data and reports to ensure the goals and objectives of the program are being met
- Knowledge of human behavior and performance for assessing and treating substance abuse disorders.
- Good communication and organizational skills
- Ability to work with a diverse population

Salary : $42,500 - $53,900
