Via Linda Behavioral Hospital, an innovative behavioral health hospital offering a full continuum of outpatient and outpatient services to the people of Scottsdale and the greater Phoenix, Arizona metro area. Our 120-bed facility will offer specialized mental health services and substance use treatment for teens, adults and older adults experiencing issues such as depression, anxiety, personality disorders and co-occurring addictions.

Job Summary:

Under the direction and supervision of the Director of Clinical Services, the Care Coordinator provides critical support and care coordination the patients of Via Linda Behavioral Hospital. The Care Coordinator is responsible for assisting and coordinating with the Clinical Services team to assure that patients have excellent and safe discharge planning, as well as providing recommendations for post-discharge support.


Qualifications

Education and Experience:

  • Associate’s Degree in behavioral health or equivalent behavioral health experience required.
  • One (1) year of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.
  • Must have or be able to obtain a Level 1 Fingerprint Clearance Card.
