Qualifications
Essential Duties and Responsibilities
- Assess perspective patients for treatment, identifying psychosocial, mental health and medical issues to determine if patient meets admission requirements for Black Bear Lodge.
- Completes Utilization Review Pre-Certifications based on thorough assessment completed by qualified Intake staff member or psychiatric evaluation completed by staff provider.
- Explains the evaluation and admission process to the potential patient/referral source.
- Documents all intake information.
- Ensures intake paperwork is complete and accurate. Creates intake files.
Education and Experience
- Bachelor’s degree in psychology, social work, or related field required, Master’s degree and/or addiction certification preferred.
- May posess current LPN and or RN license
- Minimum 1 year experience working in a psychiatric/substance abuse facility strongly preferred.
